ParisSharing information electronically is commonly written about in the future tense. We hear about the benefits that will be realised or might materialise at an unspecified time. In this set of articles E-Health Insider, in partnership with in4tek, has brought together case studies and news about developments that are actually happening in England, Wales, Northern Ireland and Canada.

Read on to find out how, for example, adult services in Bristol share information between health and social care, and how the South & East Belfast Health & Social Services NHS Trust has made integrated records available in more than 20 different service areas, serving over 1,000 staff and 120,000 patients.

In Vancouver an integrated hospital and community health management structure is being supported by integrated information. Authorities in Swansea and Torbay are well advanced in planning and rolling out integrated health and social care information systems to serve staff and clients in their areas.

The overall message from all these forward thinking services is that information sharing within organisations and between agencies is possible here and now, not tomorrow.

Change management and re-engineering the care plan in health and social care

The South & East Belfast Health & Social Services NHS Trust (SEBT) in Northern Ireland provide the full range of primary, secondary and tertiary mental health services, all community health services and social care programmes for all groups of vulnerable people. These include mental health, learning disability, physical disability, older people and children's services.

Health and social care integration in Swansea

Since starting its implementation of the in4tek Paris system two years ago the City and County of Swansea social services department has achieved considerable improvements in the delivery of integrated care services. Swansea Social Services Paris project manager, Tony Brindley, is now witnessing the evidence of the changes that have taken place.

Integration: Realising the Vision

Torbay Council's social services are midway through their implementation strategy to install the new Paris system that will support integrated health and social care services and assist the delivery of the single assessment process (SAP). The first phase, which is already live, encompasses learning disability services, with children's services and adult services scheduled to go-live before the end of the year.

PARIS leads the way in Canada

In British Columbia, Canada, the health authorities began moving towards establishing a regional healthcare system, similar in structure to PCTs in England, about five years ago.

Practitioners welcome opportunity for joint working

Bristol City Council social services department, which covers the largest unitary authority in England, serves a population of over 400,000. With an annual budget of almost £121m, the 3,000 plus staff work out of ten main offices and over 100 sites throughout the city, including eight hospitals, day centres and children’s homes. Headquarters staff and the youth offending team are based in another two offices.
